Cold medicine (multi-symptom liquid, adult) — safety profile
Moderate riskOTC multi-symptom cold/flu liquid formulation containing acetaminophen (analgesic/antipyretic), dextromethorphan (cough suppressant), phenylephrine (nasal decongestant), and propylene glycol/alcohol as vehicle.
What is this product?
OTC multi-symptom cold/flu liquid formulation containing acetaminophen (analgesic/antipyretic), dextromethorphan (cough suppressant), phenylephrine (nasal decongestant), and propylene glycol/alcohol as vehicle. CRITICAL SAFETY ISSUE: acetaminophen hepatotoxicity. Acetaminophen is the leading cause of acute liver failure in the US (~56,000 ER visits/year). Maximum daily dose is 3,000-4,000 mg; accidental overdose occurs when patients take multiple acetaminophen-containing products simultaneously. Liquid formulations increase dosing error risk.

Analgesic
- Acetaminophen (APAP) — Leading cause of acute liver failure in the US. Toxic metabolite (NAPQI) depletes hepatic glutathione. Max dose 3-4g/day. Chronic alcohol use dramatically increases hepatotoxicity risk. Antidote: N-acetylcysteine (NAC).
Antitussive
- Dextromethorphan (DXM) — NMDA receptor antagonist. Abuse potential (dissociative effects at supratherapeutic doses). Serotonin syndrome risk with SSRIs/MAOIs. DEA reports significant recreational misuse.
How to use it more safely
- Do not exceed 3,000 mg acetaminophen/day from ALL sources
- Use included dosing cup only
- Check ALL medications for acetaminophen content before combining
- Do not use with alcohol
Red flags — when to walk away
- Taking multiple products containing acetaminophen — Cumulative overdose risk — hepatotoxicity
Green flags — what to look for
- Single-ingredient products — Reduces accidental acetaminophen stacking
Safer alternatives
- Single-ingredient cold products — Target specific symptoms; reduces accidental acetaminophen overdose risk
- Ibuprofen-based cold products (no acetaminophen) — Alternative analgesic/antipyretic; no hepatotoxicity risk; GI/cardiovascular concerns instead
